All appointments are subject to approval by Divine Infusions.
Divine Infusions reserves the right to terminate any appointment at any given time.
STAFF TREATMENT: Staff will not tolerate inappropriate or disrespectful behaviour.
MANDATORY DISCLOSURE: Prior to appointments being confirmed new clients must fill in & sign a mandatory medical declaration/ waiver consent. It is important to make sure you disclose of any medical conditions/ information/ allergies/ medications/ contraindications PRIOR to your appointment. This is so that we can take into consideration any contraindications there may be & ensure this service is safe for you. If you arrive to your appointment & any contraindications have not been disclosed your appointment will be cancelled & your will forfeit your deposit. By signing the waiver you accept all risks associated with vitamin therapy via all routes (intravenous, intramuscular & subcutaneous).
DEPOSITS & CANCELLATION POLICY: A $50 deposit will be taken for all bookings. If you fail to cancel or reschedule your appointment without 48 hours notice your deposit will be forfeited. If you no-show to your appointment or are more than 15 minutes late you will also forfeit your deposit.
SCRIPTING: Our doctor will issue a script for all vitamins, nutrients, minerals & amino acids. This script incurs a fee of $50 and covers you for 12 months with Divine Infusions only.
